Sub-acute care is inpatient care designed for patients recovering from an acute illness or injury. It is a goal oriented treatment program rendered immediately after hospitalization to treat one or more medical conditions.
Sub-acute care requires the coordinated services and dedication of physicians, nurses, and rehabilitation professionals, who are trained and knowledgeable to assess and manage specific conditions.
Sub-acute care treatments are generally more intensive than traditional nursing facility care and less than acute hospital care. Patient assessment and review of the clinical treatment plan are frequently performed while the patient is rehabilitating.
