Sub-acute care is inpatient care designed for patients recovering from an acute illness or injury. It is a goal oriented treatment program rendered immediately after hospitalization to treat one or more medical conditions. Sub-acute care requires the coordinated services and dedication of physicians, nurses, and rehabilitation professionals, who are trained and knowledgeable to assess and manage specific conditions. Sub-acute care treatments are generally more intensive than traditional nursing facility care and less than acute hospital care. Patient assessment and review of the clinical treatment plan are frequently performed while the patient is rehabilitating.

Long-term care (LTC) are services we provide for people with a chronic illness or disabilities that prevent them from caring for themselves for long periods of time. It is common for long-term care to provide custodial and non-skilled care, such as assisting with average daily living tasks like dressing, bathing, and using the bathroom. Increasingly, long-term care involves providing a level of medical care that requires the expertise of our skilled practitioners to address the multiple chronic conditions associated with older populations. We offer these comprehensive services in several of our Care Centers. Long-term care may be needed by people of any age, even though it is a common needed for the elderly.

Our Assisted Living program will provide supervision or assistance with activities of daily living (ADLs); coordination of services by outside health care providers; and monitoring of resident activities to help to ensure their health, safety, and well-being. Assistance may include our administration or supervision of medication, or personal care services provided by our trained staff person.

We provide 24-hour care in a separate and secure environment, complete with dining rooms, outdoor courtyards and amenities. A highly trained, accommodating staff provides our residents with personalized attention, nutritious meals and stimulating activities to meet each individual’s abilities. Familiar, structured routines ensure that our residents are comfortable throughout their day. Surrounded by familiar faces and places, they enjoy a warm and inviting home. Some of the therapeutic activities include social, educational, devotional and recreational programs. We have specially trained staff to meet the needs of Alzheimer's residents available up to 24 hours a day. Arrangements can be made to, offer private or semi-private living spaces designed to help Alzheimer's residents function as independently as possible.

Our Ventilator Care team is led by Board Certified Pulmonologists focused on taking an aggressive, multi-disciplinary approach to ventilator rehabilitation. Each team’s superior clinical expertise enables them to design individualized health plans that fit within each patient’s comprehensive respiratory, physical, occupational and speech therapy treatment programs. In some cases, these physician-directed programs are focused on reducing or eliminating a patient’s dependence on a ventilator for breathing. In others, the programs are designed to provide optimum care for ventilator dependent patients as a result of injury, neurological disorders or pulmonary disease.

Hospice Care is specialized health care services that focus on pain relief, symptom control, comfort and support for terminally ill patients and their families. Our Hospice care services include physical, emotional, and spiritual support during this time.

Hospice care services are provided by regional hospice agencies which are licensed by the New Jersey Department of Health and Senior Services and are certified to participate in the Medicare program. These agencies work in conjunction with the facility staff to assist the patients who no longer wish to continue with such active treatments and understand that there is limited life expectancy.

Delivering all levels of occupational, physical and speech therapy, our Rehabilitation Services clinical staff is an integral part of our organization. Specialized training enables our therapists to provide care to patients and residents with complex diagnoses, helping them to overcome deficits that result from illness, disease, disability, injury, surgery or the aging process. Our individually designed therapy services enhance walking, muscle function, coordination, endurance, and mobility; educate and train patients and their caregivers in daily living skills (such as dressing, bathing, eating, and grooming) provide therapeutic activities for memory and comprehension; and assist with verbal and non-verbal communication skills.
